Cigna and Blue Cross/Blue Shield (including Blue Choice) are accepted. Your copay is due at the time of service.

Other types of insurance often will reimburse you for your treatment. At the end of your visit you will be provided with an itemized statement which you can submit. The full amount is due at the time of service.

Refills, forms, and letters requested outside of an appointment will incur a $20 service charge.
